Summary or Abstract : The purpose of this study was to investigate the ability of the MCMI-III Modifying Indices to identify persons engaged in gross symptom malingering. The sample consisted of 202 psychiatric inpatients and 230 college undergraduate students. The students were randomly assigned to one of two groups, a fake-bad (Fake-Bad) condition or an honest (Standard Instructions) responding condition. Analyses investigated the ability of the MCMI-III Modifier Indices to correctly discriminate students in the Fake-Bad group from the Psychiatric Inpatient reference group. The sensitivity and specificity rate of the published MCMI-III Scale X cutoff score to detect malingering was determined. The rates of classification for the published MCMI-III Scale X cutoff score (Scale X > 178) was moderate, providing an overall diagnostic power of 63.9 . Optimal cutoff scores were computed for all three of the Modifier Indices to improve the sensitivity of the MCMI-III to detect malingering. A discriminate analysis found that the MCMI-III provided significant differentiation between groups. Finally, ROC analyses of the three Modifier Indices were completed. The ROC analyses demonstrated that Scale X provided the best overall rates of classification. Overall, the MCMI-III Modifier Indices provided moderate effectiveness in differentiating between individuals engaging in gross symptom malingering and psychiatric inpatients..
