Summary or Abstract : Community health worker programs are relatively new approaches to the management of chronic diseases. These programs have the potential to improve health outcomes and decrease health care expenditures. There is increasing evidence that community health workers improve outcomes in clients with diabetes.The aim of this evidence based practice project was to analyze the research literature on the effects of community health workers on the outcomes of HbA1c, blood pressure, BMI, lipids, and diabetes knowledge for people with type 1 or type 2 diabetes. The effect of the community health worker intervention was compared to standard provider care.Computerized searches were conducted and found 3 systematic literature reviews, 1 quasi-experimental study, and 2 descriptive studies. In all, 19 studies were synthesized for this project. While the use of the community health worker programs showed promising outcomes, the existing evidence is insufficient and additional research is needed to determine its efficacy. In the future, community health worker interventions should be standardized creating consistent programs with consistent outcomes, and training for the community health workers..
